 Weight Wise Clinic
This notice describes how medical information about you may be used and disclosed, how you can get access to this information, and your rights with regards to your Protected Health Information, as set forth by the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
Weight Wise uses the highest levels safety possible to ensure the privacy of your protected health information. We document each visit within your record, as well as, each corresponding payment for those visits. The information we collect is used to provide you with the best care possible. This information will be documented within your personal chart, and within our computer systems. We are required by law to maintain the privacy of your protected health information.
You have the right, at any time, to request a copy of our complete Notice of Privacy Practices. Each Weight Wise location will have a copy of the complete Notice of Privacy Practices at the front desk. The complete Privacy Notice explains:
-How we may use and disclose your protected health information.
-You rights with regards to this information, as well as how you may exercise these rights. This also includes how to submit your complaints about our practices with respect to your protected information.

This Notice of Privacy Practices applies to every record that is generated by our physicians and/or staff at each one of our clinics. The Notice of Privacy Practices applies to each Weight Wise clinic in Moss Bluff; Lake Charles; Kinder; DeRidder; Jennings; and Scott, LA.
