       remaining questions you may have!




Step Two:
 



   You may now create an online account that will allow you to fill out and submit the history forms online. You
   will also have access to all of our online resources and services, as well as future enhancements! All of the
   safety measures we  have in place either  meet, or exceed  all of the requirements  set forth by the  Health
   Insurance Portability and Accountability Act of 1996. To learn more about the security of our program, click

If you prefer  not to use  our secure online  features and  forms, and would  rather fill out the forms manually – we ask you to please come in at least 20-30 minutes before your scheduled appointment time. This will allow enough time for you to fill out the  forms and complete our  new patient processing.
 
Note: Please be sure to arrive on time for your appointment. We want to provide you with the highest level of care possible for the entirety of your time in our program, at each and every visit to our clinics.
